Introduction
ATK Services & Marketing , which can be referred to as "we" "our" or "us", provides an electronic personal health record service through the use of our mobile Application “ iSEHATI” as a free service to patients (and their personal representatives), with agreements with doctors and other healthcare providers within our healthcare offerings network. This Mobile Application User Agreement (this "Agreement") applies to your use of the “iSEHATI” Application. By signing up for, or otherwise obtaining, an account, or by accessing or using the “iSEHATI” Apps, you are entering into this Agreement and agreeing to be bound by its terms. Please read this Agreement carefully, and do not sign up for an account or use the iSEHATI Application if you are unwilling or unable to be bound by this Agreement.
The iSEHATI mobile Application
iSEHATI allows you to retrieve and store certain health information that is available to you by your healthcare providers. Moreover, you can access the health information of another individual, such as your child, granted that you have the authority for such access.
To access your healthcare records from any of the healthcare Providers visited , you initially required to register an account and have an existing medical records with a healthcare Provider that would grant you access to your health medical records. To register, you will need to satisfy our identity verification and certification procedures. To access your health records from each additional healthcare provider visited, The provider must grant you that access.
Your Health records are created by your healthcare Provider, such information may contain typographical errors, inaccuracies or omissions. In addition, your health information that is retrieved and stored in iSEHATI application does not necessarily display all information in the health records retained by your healthcare Provider. If you think that your medical information displayed in the iSEHATI application is inaccurate or incomplete, or if you would like to request a complete copy of your medical records, please contact your healthcare Provider directly. The information stored in iSEHATI is referenced to the creation source, that is your healthcare Provider, and it is part of your Provider's health records about you, therefore, you cannot delete such information. We do not control your Provider's use or disclosure of your health information. . If you wish to restrict the disclosures that your Provider makes of your health information, please contact your Provider directly. Your healthcare Provider also retains the ability to revoke your access to your medical records.
The purpose of this Policy is to describe how we collect, use, and share information about you with respect to the Services covered by our Terms Of Use and not to any other service we may offer to any other individual or customer. When we store, process or transmit "individually identifiable health information" on behalf of a healthcare provider who entered into a HIMMA Cloud User Agreement, we do so as its "business associate”. Under this agreement, we cannot use or disclose individually identifiable health information in a way that the provider itself may not.
Definitions
“De-Identified Personal Information” means Personal Information from which all identifiers that could reasonably be anticipated to identify an individual by an anticipated recipient – such an individual’s name, contact information, or government identifiers – have been encrypted.
Information Collected by Our Services
We collect information from you when you:
- Register to an account.
- Enter information on our Services, such as when you request an appointment from a healthcare provider.
- Upload a document, image, or other data file on our Services;
Information we collect about you may include your name, address, telephone number, email address, or the information you enter on or upload to our Services.
We also collect information on your behalf, such as when you register for an account and your healthcare provider updates your health information that you may access through your iSEHATI account. However, we store such information in a de-identified personal information manner.
We automatically gather information whenever you log in, or otherwise interact with our Services to enhance and operate our Services in a number of ways, such as to:
- Save user preferences and information;
- Preserve session settings and activity;
- Authenticate users;
- Enable support and security features;
- Tailor the delivery of informational messages; and
- Analyze the performance and use of our Services and its various features and content.
Device Information:
When you interact with our Services, we collect information about your Device such as the URL of services your Device is requesting and the referring web pages, your IP address, Device type, operating system, browser type, application identifier, and, under certain circumstances, the location information your Device sends to us.
How We Use Information
We may use the information we collect for the following purposes:
- Operating our Services and developing new functionality and features;
- Responding to questions and communications, or obtaining your feedback about our Services;
- Preparing and delivering announcements about features, functionality, terms of use, or other aspects of our Services;
- Providing you with more relevant content, including medical-related information your healthcare provider may share with you through the Services;
- Analyzing usage trends and patterns and measuring the effectiveness of the Services and its features;
- Safeguarding and protecting our Services, the information we collect, and the rights of us, our users or third parties, and in response to legal process;
- • Any other purpose described in this Policy and/ or the Terms of use; or When we otherwise have your permission.
Incorporation by Reference
Without limiting the generality of the foregoing, this Agreement incorporates by reference the provisions of our Terms Of Use with all references to the phrase "our Services" whenever and wherever is applicable
